Job summary

Volunteers of America Los Angeles is seeking a Case Manager Generalist to provide on-site case management for low-acuity clients. You will coordinate services, conduct comprehensive screenings, and develop individualized care plans to meet short- and long-term needs.

Responsibilities include advocacy before agencies, coordinating care through a team, and connecting clients with resources to improve outcomes in a supportive environment.

Qualifications

• Must pass fingerprint clearance and background check.
• Minimum 1 year of general experience in case management or a related field; or a Bachelor’s degree in Social Services.
• Preferred: Bachelor’s Degree in Social Work or related field; HMIS navigation experience.

Responsibilities

• Conducts screenings and assessments to collect client information and assess barriers.
• Develops and monitors individualized case plans and coordinates services.
• Advocates for clients with judicial, social service, and community bodies.
