Senior TLC is seeking an On-Site Specialist Liaison in Shelby, NC to coordinate visits for on-site specialists and manage follow-up with providers and patients. The role reports to the HIM Manager and requires strong collaboration with clinical teams.

Responsibilities include preparing visit documentation, coordinating scheduling, and maintaining patient charts in Elation. The ideal candidate will be detail-oriented, discreet with patient information, and capable of adapting to changing

Qualifications

• High school diploma or GED required.
• Certification/registration as Medical Office Assistant, CMA, RMA, CCMA, CNA or LPN.
• Excellent communication and interpersonal skills; teamwork is essential.
• Ability to handle confidential information with discretion.
• Familiarity with EMR systems and scheduling tools.
• Experience in a medical/geriatric office setting preferred.

Responsabilités

• Monitor service tracking for on-site referrals and schedule visits.
• Prepare and send documentation to on-site providers within required timeframes.
• Schedule on-site appointments and coordinate transportation if needed.
• Notify participants and caregivers about upcoming visits in advance.
• Upload consent forms and visit notes to patient charts and share with PCPs.
• Maintain on-site calendar and update SharePoint calendars regularly.
• Support mission to improve seniors' quality of life and care coordination.
