Kintegra Health is seeking an On-Site Specialist Liaison to prepare for and coordinate upcoming on-site specialist visits. This role interfaces with on-site providers and referring physicians, ensuring timely documentation and follow-up within the HIM department.

Qualifications

• Education requires a high school credential and related certifications.
• Experience with medical office settings and elderly care preferred.

Responsibilities

• Monitors service tracking for all on-site referrals and creates schedules.
• Prepares documentation and forwards to on-site providers within deadlines.
• Sets up day center spaces for on-site visits when needed.
• Communicates upcoming appointments to participants and caregivers.
• Obtains and uploads consent forms to patient charts.
• Schedules on-site appointments and updates calendars.
• Provides same-day notes to providers and routes to PCP for review.
• Reschedules missed visits and coordinates monthly on-site visits.
