Job summary

Colorado Coalition for the Homeless is seeking a Patient Navigation Specialist - Vision to help patients access comprehensive vision services and overcome barriers to care. You will act as the front desk liaison, verify insurance, schedule appointments, and assist with transportation and referrals.

The role supports trauma-informed, patient-centered care within the Eye Clinic, coordinating with ophthalmic staff and external providers to ensure timely access to services for people experiencing

Qualifications

• High School diploma or equivalent required.
• One year of experience in a healthcare setting, preferably eye care, preferred.
• Experience with insurance enrollment, healthcare front desk operations strongly preferred.

Responsibilities

• Serve as the first point of contact for patients, clients, visitors, and vendors with a professional, trauma-informed and culturally responsive approach.
• Complete client registration, verify insurance, create health records, schedule appointments, and enter data into the EHR.
• Verify insurance status and connect clients to enrollment resources to ensure access to care and affordability.
• Assist patients with scheduling eye exams and referrals; arrange transportation and accompany to appointments as needed.
• Escort patients to specialty referrals for procedures; support ophthalmic services and basic testing.
• Assist with optical duties (eyeglasses fitting/adjustments) and enter orders into the system.
• Coordinate care with the eye clinic team and document follow-up and treatment compliance.
