Job summary

Northwestern Medicine is seeking a Transplant Nurse Coordinator based in Illinois. The role serves as the primary coordinator for deceased donor transplant activity and assumes 24/7 responsibility for organ offers, logistics and multidisciplinary communication during assigned shifts.

Under transplant physicians, you will coordinate care through all phases, refer to external clinicians when appropriate, and comply with departmental protocols and regulatory standards.

Qualifications

• BSN required
• Licensed RN in Illinois
• BLS certification

Responsibilities

• Plan, direct, provide and document patient care in designated setting
• Coordinate comprehensive care including consent, data collection, referrals and follow-up
• Collaborate with interdisciplinary team to optimize patient wellness
• Educate patients, families and staff and serve as a resource for education
• Act as communication link between patients, families, team and external hospitals/agencies
• Manage organ offers in DonorNet from notification to transplantation
• Work on call with physician team to coordinate logistics of organ acceptance
• Ensure required consents are obtained prior to organ acceptance
• Handle off-hours and overnight shifts as needed
• Prioritize multiple tasks in a fast-paced environment
