Qualifications

• Bachelor of Science in Nursing (BSN) required.
• Licensed Registered Nurse (RN) in the state of Illinois.
• CPR-Basic Life Support (BLS) required.
• Minimum 2-3 years of recent & related clinical experience.

Responsibilities

• Planning, directing, providing, and documenting appropriate care for patients in the designated care setting.
• Coordinating comprehensive care including consent, data collection, follow-up visits, referrals, testing and long-term follow-up.
• Collaborating with the interdisciplinary team to maximize wellness and patient outcomes.
• Educating patients, family, caregivers, and staff based on personalized needs.
