Job summary

Sierra Meadows Behavioral Health is seeking a Registered SUD Counselor in Fresno to provide assessments, treatment planning, and ongoing client support. The role emphasizes coordination with the Program Manager, crisis response, and discharge planning.

Schedule is Monday through Friday, 8:30am to 5:00pm. Requires High School Diploma or GED, a DHCS credential, and 1-3 years as a case manager; travel may be involved and on-call duties as needed.

Qualifications

• Strong leadership, communication, and organizational skills.
• Advanced assessment, treatment planning, discharge planning, and documentation skills.
• Knowledge of dual diagnosis treatment practices and diverse populations.

Responsibilities

• Complete admissions/intakes for new clients and assess level of care.
• Develop and implement treatment plans and aftercare recommendations.
• Document progress notes and group notes; ensure compliance with DHCS and other regulations.
• Coordinate admissions and discharge with the Program Manager and communicate with families and referral sources.
• Respond to crises and participate in site visits and trainings.
