Job summary

Sierra Health and Wellness is looking for a dedicated Care Coordinator - SUD Counselor. The position involves assessing clients' needs related to substance abuse and developing treatment plans. Responsibilities also include running group therapies, providing crisis management, and coordinating care.

Applicants must have a High School diploma or GED, certification as a drug and alcohol counselor, and a minimum of one year of experience. A valid driver’s license and insurability is required.

Qualifications

• Must possess a High School diploma or GED.
• Certification as a Drug and Alcohol Counselor by a California Regulatory Agency.
• Minimum one year of experience in Drug and Alcohol counseling.
• Familiar with HIPAA/client confidentiality and personal rights.
• Possess a valid license in drug and alcohol counseling.
• Current CPR and First Aid certification.

Responsibilities

• Conduct assessments and participate in treatment planning with clients.
• Facilitate discharge planning and community outreach.
• Respond to emergencies and client complaints.
• Meet clients individually at least twice a week.
• Complete notes for every group and interaction.
