Job summary

Northwestern Medicine is seeking an On-Call Social Worker for the Mobile Crisis Response Team. This part-time role is stationed at home unless called in and requires arrival within 30 minutes. Hours are 8:00 PM–8:30 AM, with 3-8 shifts per month including weekends and holidays.

You will provide high-quality social services to patients and families in inpatient and outpatient settings, coordinating care with physicians and staff, with trauma-informed crisis stabilization in emergency settings.

Qualifications

• Bachelor's with major in social work or related field.
• Documented experience providing direct service to patients.
• Knowledge of the mental health code.
• Ability to work in a multidisciplinary team.

Responsibilities

• Provide high quality social services to patient and family to promote optimal functioning.
• Deliver direct services to patients and families in inpatient and outpatient settings.
• Coordinate options and services with physicians and healthcare personnel.
• On-call social services for Emergency Department patients and other units as needed.
