Job summary

St. Croix Hospice in Frederic, WI is seeking a Registered Nurse On-Call to provide compassionate after-hours care within our 24/5 model. You will respond to triage needs, complete after-hours admissions, and deliver clinical expertise with empathy for patients and families.

This role offers a 7 on / 7 off overnight schedule, requires an RN license, CPR certification, and 1+ year of acute care experience. You will develop care plans, administer medications, and maintain accurate clinical

Responsibilities

• Respond to triage calls and provide after-hours nursing visits.

• Develop and adjust individualized care plans as needs evolve.
• Administer medications and educate patients and caregivers.

In the role of Registered Nurse On-Call, you’ll be a trusted presence during evenings, nights, and weekends within our 24/5? care model. You’ll respond to urgent needs, complete admissions after hours, and provide clinical expertise paired with genuine compassion. Qualifications: Registered Nurse On-Call Requirements & Responsibilities:

• Graduate of an accredited nursing program (Associates, Bachelors, or Masters)
• 1+ years of recent acute care experience required; hospice or home care experience preferred
• Current RN license in state of practice and CPR certification
• Respond to triage calls and provide after-hours nursing visits for symptom management, admissions, and care plan updates
• Develop and adjust individualized care plans based on evolving needs
• Administer medications and treatments as prescribed, offering education and support to patients and caregivers
• Maintain timely and accurate clinical documentation of care, outcomes, and communication
• Must have reliable transportation, valid driver’s license, and proof of auto insurance
