The Christ Hospital Health Network is seeking a Registered Nurse for an On-Call role to support a team of nurses and patients, providing after-hours care in patients' homes. You will deliver one-on-one care, work at the top of your license, and participate in an interdisciplinary team focused on compassionate, quality outcomes.

Responsibilities include OASIS, care planning, medication administration, chart reviews, and timely documentation.

Formación

• Active RN license in the state of employment.
• Minimum one year RN experience in an acute care setting; home care experience preferred.
• Strong communication and time-management abilities.
• Ability to work autonomously within a multidisciplinary team.

Responsabilidades

• Respond to after-hours patient needs as part of a team of nurses and patients.
• Provide one-on-one patient care and work at the top of your license.
• Administer medications as prescribed and prioritize visits for high-risk patients.
• Oversee care by nurse aides and LPNs and coordinate admissions, transfers, and discharges.
• Complete clinical documentation and SOC/ROC/Recert within 24 hours.
• Collaborate with physicians to ensure optimal patient care.
