Job summary

Community LIFE is seeking an On-Call Registered Nurse to provide after-hours and weekend care in the McKeesport service area. The role offers full-time pay for working every other week, with on-call shifts from 3:30 PM to 8:30 AM.

You will support participants in their homes and collaborate with an interdisciplinary team to ensure continuity of care. Requirements include graduation from an accredited nursing program and a PA driver's license; 1 year of frail/elderly care experience is preferred.

Qualifications

Graduate of an accredited nursing program.
Valid PA driver's license.
Minimum 1 year of frail/elderly care experience (home care, long-term care, or community health).
Experience in patient and caregiver education is a plus.

Responsibilities

Provide after-hours nursing care and support to older adult participants.
Assess and manage episodic illnesses in collaboration with the on-call provider.
Deliver prescribed treatments and nursing interventions.
Educate participants and caregivers on care plans and health management.
Document all care and changes in condition promptly and accurately.
Coordinate care and communicate effectively with the interdisciplinary team.
