Job summary

Community LIFE is seeking an On-Call Registered Nurse to join our home-based care team. You will be paid full-time wages while working every other week, providing after-hours and weekend care to participants in Wilkinsburg and Homestead service areas.

The role includes overnight on-call shifts, patient and caregiver education, and coordination with the interdisciplinary team to ensure continuity of care for older adults living at home.

Qualifications

• Graduate of an accredited nursing program.
• Minimum 1 year of experience with frail or elderly populations (home care, long-term care, or community health).
• Valid PA driver’s license.
• Experience in patient and caregiver education is a plus.

Responsibilities

• Provide after-hours nursing care and support to older adult participants.
• Assess and manage episodic illnesses in collaboration with the on-call provider.
• Deliver prescribed treatments and nursing interventions.
• Educate participants and caregivers on care plans and health management.
• Document all care and changes in condition promptly and accurately.
• Coordinate care and communicate effectively with the interdisciplinary team.

Schedule

OvernightWeekdays: On-Call from 3:30 PM – 8:30 AM (Monday–Friday) EVERY OTHER WEEK ONLY
