Job summary

Horizon House, Inc. is seeking an On-Call Rehabilitation Counselor to join the Behavioral Health Services Division.

The role focuses on care and supervision to clients, supporting growth toward independence in self-care, daily living, socialization, and community living, with involvement in individual service plans and related documentation. Responsibilities include guiding clients in nutrition and self-care, updating service plans, ensuring safety, and completing medication training in line

Qualifications

• High school diploma or equivalent; associate’s degree preferred in social sciences or health care-related field with one year MH/MR experience, or equivalent.
• Good oral and written communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Provide guidance and support to clients; teach and reinforce skill development in nutrition, self-care, routine medical care, and follow-up.
• Review and modify individual service plans to reflect growth and development.
• Ensure the safety of assigned individuals by maintaining a hazard-free environment.
• Complete medication training and manage medications per self-medication plans and regulations.
