Job summary

Horizon House, Inc. in Philadelphia is seeking a Full-Time Rehabilitation Counselor to provide care and supervision for assigned clients and promote growth toward independence in self-care, daily living skills, socialization, and community living.

You will participate in developing individual service plans and document related plans. In this role you will guide clients, review plans, ensure safety, and support medication training aligned with state regulations.

Qualifications

• A.A. degree in the MH/MR field or health care-related field with at least 1 year of relevant experience or equivalent combination.
• Strong written and oral communication skills.
• Ability to work independently and as part of a team.

Job description

We are seeking a Full-Time Rehabilitation Counselor to work in our Behavioral Health Services Division. To provide care and supervision for all assigned clients and ensure a safe, healthy, secure, and pleasant environment that promotes growth and development. Facilitate client growth and development toward independence in self-care, daily living skills, socialization, and community living. Participate in developing individual service plans and related documentation.

Other Responsibilities, but not all inclusive;

• Provide guidance and support to all clients; teach and reinforce skill development in the areas of nutrition, basic self-care, routine medical care, and follow-up, as developed through the joint planning sessions of the individual service plans.
• Review and modify individual service plans to accommodate growth and development.
• Ensure the overall safety of assigned individuals by maintaining a hazard-free environment and avoiding potentially dangerous situations.
• Successful completion of medication training. Maintain and monitor medications consistent with individuals’ self-medication plans, in compliance with State and Agency medication administration regulations.

Qualifications

• A.A. degree in the social sciences or a health care-related field and one year of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required.
• Good communication skills, both oral and written.
• Ability to work independently and as part of a team.

Scope

• Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend, which may include direct intervention with residents.
• Possess manual dexterity and fine motor skills.
• May be required to possess a valid driver's license and an acceptable driving record (contingent upon facility location and client and program needs).
• May require crisis intervention in emergencies.
• May be required to report to work during emergencies, including inclement weather.
