Horizon House, Inc. in Lansdowne, PA, is seeking a Full-time Rehabilitation Counselor to work within the Developmental Services Division. The role focuses on caring for residents and promoting growth toward independence in daily living and community integration.

Responsibilities include developing individual service plans, documenting client progress, and collaborating with a multidisciplinary team to ensure a safe, supportive environment and quality service delivery.

Qualifications

• A.A. degree in the social sciences or health care related field and one year of relevant MH/MR experience.
• Crisis intervention and de-escalation skills required.
• Good oral and written communication skills.
• Ability to work independently and as part of a team.
• Required to possess a valid drivers' license and an acceptable driving record.

Responsibilities

• Provide care and supervision of all assigned residents to ensure a safe, healthy, secure and pleasant environment, which promotes growth and development.
• To facilitate client growth and development toward independence in self-care, daily living skills, socialization and community living.
• To participate in the development of the individual service plans and related documentation and document client progress as they move through the service delivery system.
