Adaptive Home Health in Detroit is seeking an On Call Registered Nurse (RN) – Case Manager for part-time, after-hours field-based work. You will perform comprehensive in-home assessments, focused wound care, and skilled documentation while coordinating care with physicians and supervising support staff.

The role emphasizes responsiveness to discharge planners, allied professionals, and families. Active Michigan RN license required; home health/OASIS experience preferred.
