Constellation Health Services seeks a compassionate RN to provide hospice care for patients at home, in facilities, and in skilled nursing settings. The role includes on-call shifts from 5pm to 8:30am with a 7-on/7-off schedule. Candidates must be a graduate of an accredited RN program and licensed in the state. Hospice experience is preferred, with strong documentation and interdisciplinary coordination.

Qualifications

• Proficient with basic clinical documentation and care coordination.

Constellation Health Services is looking for a compassionate and skilled Registered Nurse to work with hospice patients and their families in their homes, assisted living facilities, and in skilled nursing facilities. A common schedule will be from 5pm to 8:30am with 7 days on and 7 days off. The On-Call RN is responsible for ensuring that care and services are delivered appropriately.
