Job summary

Constellation Health Services is seeking a compassionate Hospice Registered Nurse Case Manager to provide skilled nursing care in patients’ homes, assisted living facilities, and skilled nursing facilities in New Jersey. The role emphasizes rapid assessment, care coordination, and interdisciplinary collaboration to ensure high-quality, compassionate end-of-life care.

The successful candidate will have an accredited RN program degree, an active RN license in New Jersey, and 1–2 years of RN

Qualifications

• Graduate of an accredited RN program.
• Registered Nurse licensed in the state of employment.
• 1–2 years of RN experience required; hospice experience strongly desired.
• Basic knowledge of iOS operating system.

Responsibilities

• Provide skilled nursing care including medication administration as prescribed.
• Assess and coordinate the plan of care with disciplines and family/caregivers.
• Observe and report changes to nursing supervisor and physician.
• Participate in weekly Interdisciplinary Team Meetings and update on patient status.
• Maintain documentation per agency standards in electronic system.
• Provide input to Home Health Aide Supervisor for hospice aide evaluation.
• Weekend and holidays coverage as needed.
