Job summary

Constellation Health Services seeks a compassionate RN to provide hospice care for patients at home, in facilities, and in skilled nursing settings. The role includes on-call shifts from 5pm to 8:30am with a 7-on/7-off schedule. Candidates must be a graduate of an accredited RN program and licensed in the state. Hospice experience is preferred, with strong documentation and interdisciplinary coordination.

Qualifications

• Graduate of an accredited RN program.
• Licensed RN in the state of employment.
• 1–2 years RN experience; hospice experience preferred.
• Proficient with basic clinical documentation and care coordination.

Responsibilities

• Provide skilled nursing care including med administration per plan.
• Perform comprehensive patient assessments and coordinate care with the interdisciplinary team.
• Attend weekly team meetings and communicate patient status.
• Maintain documentation per agency standards.
• Support hospice aides and communicate with family caregivers.
