The role includes assessing patients, coordinating care with the interdisciplinary team, documenting accurately, and supporting families. Weekend/holiday coverage is required; competitive benefits are offered.

Qualifications

• Graduate of an accredited RN program.
• RN licensed to practice in the state of employment.
• 1-2 years RN experience in hospice or home health preferred.

Responsibilities

• Provides skilled nursing care in patients' homes, assisted living facilities, and SNFs as prescribed.
• Performs comprehensive assessments and coordinates plan of care with interdisciplinary team.
• Monitors patient status and reports changes to nursing supervisor and physician.
• Documents care using the agency's electronic system in a timely manner.
• Participates in weekly interdisciplinary team meetings; supports hospice aides and family caregivers.
• Weekend and holiday coverage as needed.
