SALMON Health and Retirement is seeking an After-Hours On-Call Private Duty Aide to provide dependable, flexible care in clients' homes in Medway, MA. This role covers last-minute and urgent needs outside normal hours, ensuring continuity of care per each client's plan.

Qualifications

• CNA or HHA certification preferred or required based on agency and client needs.
• Current CPR certification, if required by agency policy.
• Demonstrated reliability and ability to work independently in clients' homes.
• Strong communication and interpersonal skills.
• Reliable transportation and ability to travel throughout the assigned service area.

Responsibilities

• Provide personal care and assistance with activities of daily living in accordance with the client's established plan of care.
• Assist clients with bathing, dressing, grooming, toileting, mobility, transfers, ambulation, and other approved personal care needs.
• Provide companionship, supervision, and safety monitoring as required by the client's plan of care.
• Assist with meal preparation, light housekeeping, laundry, and other approved homemaking responsibilities.
• Provide medication reminders when permitted by agency policy and the client's plan of care.
• Report changes in a client's condition or safety concerns to the supervisor promptly.
