Job summary

Westminster Communities of Florida in Bradenton is seeking a licensed Physical Therapist to join its Health Center team. You will assess residents, plan care, deliver treatment, and contribute to clinical program development.

Responsibilities include evaluation and treatment to restore mobility, performing home assessments during discharge, and conducting routine screens. Florida PT license and at least one year in long-term care preferred.

Qualifications

• Education: Current State of Florida Physical Therapist license.
• One year of long-term care desirable. Med-surg, Home Health experience helpful.
• Working with handicapped or developmental disabled, or older adults preferred.

Responsibilities

• Evaluation and treatment of physical conditions to restore functions and improve mobility as ordered by MD; design and execute Care Plan.
• Conducting assessment and screens to determine the need for therapy; provide modalities including electrical stimulation, heat/cold packs and therapeutic massage.
• Maintain timely and accurate documentation, including updating information in the systems.
• Performing Resident Home Assessments as part of the discharge process.
• Performing Resident's screenings during admission, and quarterly/annual screenings to determine the need for therapy.
