Job summary

Qualifications

• At least one year of experience in pre-hospital or post-hospital settings.
• Current CPR certification prior to start date.
• Bachelor’s degree in healthcare-related field preferred.
• Understanding homelessness and related health/social needs.
• Ability to work in a fast-paced environment and with diverse populations.

Responsibilities

• Provide intensive case management for Recuperative Care participants.
• Welcome, orient, engage participants daily and coordinate resource needs.
• Link participants to medical services and housing resources as part of care planning.
• Coordinate with housing specialists and health providers to support housing goals and access to services.
• Perform welfare checks, document interactions, and maintain EMR updates.
• Attend interdisciplinary meetings and participate in case reviews to optimize care.
