Job summary

Central City Concern seeks a Health Assistant to join the Old Town Clinic team in Portland. You will support a multi-disciplinary primary care team serving homeless and low-income patients, ensuring timely communication, documentation and outreach tied to health services.

The role requires a high school diploma or GED (Bachelor’s in health or social preferred), CPR certification, and ability to pass required screenings. Spanish fluency is a plus and union protections apply.

Qualifications

  • High school diploma or GED required.
  • Bachelor’s degree in health or social preferred.
  • Experience with homeless, low-income populations or vulnerable groups.
  • Spanish language fluency preferred.
  • Must have CPR certification before start date.
  • Must pass pre-employment drug screen, TB test and background check.

Responsibilities

  • Receive, document and direct patient calls according to clinic protocols.
  • Electronically file and index clinical information in EMR.
  • Assist Care Team Managers and providers with health services.
  • Provide follow-up contact to patients identified by CTM or care team members.
  • Participate in outreach to engage patients for preventive services and chronic disease management.
  • Provide educational information to patients about available services.
  • Coordinate services with care team and client’s support network.
  • Maintain client records per program standards and funding requirements.
  • Attend mandatory CCC trainings and comply with privacy regulations.
