Job summary

Umpqua Valley Nursing & Rehabilitation Center, part of the Volare Health family, is seeking compassionate LPNs to join our care team in Oregon.

PRN shifts available, 1–2 per week, with weekend requirements; you will assess residents, develop care plans, supervise CNAs, and ensure regulatory compliance.

Pay is $39.11–$62.15 per hour based on licensed experience; all qualified applicants encouraged to apply.

Qualifications

• Current, active LPN license in Oregon.
• 1–2 years of licensed nursing experience preferred.
• Experience in SNF or long-term care is preferred.

Responsibilities

• Assess resident needs and provide appropriate nursing care.
• Develop and implement individualized care plans.
• Supervise CNAs and other care team members.
• Follow all state and federal regulations, facility policies, and standards of nursing practice.
• Collaborate with the interdisciplinary care team to support resident health, comfort, and quality of life.
• Build meaningful relationships with residents and their families through compassionate, respectful care.

What You’ll Do

As an LPN, you will play an important role in providing safe, compassionate, and individualized care to our residents. Responsibilities include:

Assessing resident needs and providing appropriate nursing care.

Developing and implementing individualized care plans.

Supervising CNAs and other care team members in the delivery of nursing services.

Following all applicable state and federal regulations, facility policies, and standards of nursing practice.

Collaborating with the interdisciplinary care team to support resident health, comfort, and quality of life.

Building meaningful relationships with residents and their families through compassionate, respectful care.

What We’re Looking For

Current, active LPN license in the state of Oregon.

1–2 years of licensed nursing experience preferred.

Skilled nursing facility (SNF) or long-term care (LTC) experience preferred.

Ability to assess resident needs and provide appropriate nursing interventions.

Experience developing and following individualized care
