Job summary

You will address urgent patient needs, collaborate with the RN Case Manager and IDT, and document thoroughly to ensure high-quality care.

The role offers full-time hours, with 7 on/7 off schedule or described shifts, and supports a mission-driven, patient-centered approach in a supportive team environment.
