Grace Healthcare Services is seeking an On Call Visit Nurse (RN) to provide compassionate hospice care in Ocean and Monmouth Counties. This full-time role supports patients and families with urgent needs, under the supervision of an RN Case Manager and within the IDT framework.

The ideal candidate holds an active RN license, hospice or palliative care experience is valued, and thrives in a fast-paced, mission-driven environment with strong communication and organizational skills.

Responsibilities

• Provide compassionate, on-call care to hospice patients, addressing urgent needs with empathy and professionalism.
• Collaborate with the RN Case Manager to implement tailored interventions.
• Maintain accurate documentation for high-quality care.
• Be a vital member of the Interdisciplinary Team (IDT) contributing to a comprehensive plan of care.
• Work under supervision of a Registered Nurse to assess and meet patient and family needs.
• Provide continuous, compassionate care in alignment with hospice standards.
