Job summary

Grace Healthcare Services is seeking a Hospice Registered Nurse (RN) to join our Interdisciplinary Team in Monmouth County, New Jersey. The role involves assessing and addressing the comprehensive needs of terminally ill patients and their families, with an emphasis on compassionate, high-quality care.

The RN will initiate interventions on admission and maintain a continuous plan of care, traveling to patient locations as assigned while collaborating with physicians, social workers, and other

Qualifications

• New Jersey or Multi-State RN license.
• Minimum of two (2) years nursing experience in acute-care settings such as medical, surgical, or oncology units.
• Strong clinical practice with a focus on customer service, excellent communication abilities, and the capacity to work independently in a fast-paced environment.

Responsibilities

• Deliver skilled nursing care per individualized care plans, ensuring comfort and quality of life for terminally ill patients.
• Collaborate with physicians, social workers, chaplains, nursing assistants, and other stakeholders to develop and adjust patient care plans as needed.
• Educate and support patients' families and primary caregivers, providing guidance on care techniques and offering emotional support.
• Serve as a liaison between patients, families, and other healthcare providers, ensuring clear communication and advocating for patients' wishes and needs.
