Job summary

MJHS Hospice and Palliative Care program seeks an on-call Registered Nurse to respond to after-hours patient visits, coordinating with the on-call manager to ensure safe and effective plans of care. Responsibilities include education on meds, patient safety, and symptom management, with emotional support to families during patient deaths.

The role requires a valid RN license and prior experience in Hospice or related fields; CHPN certification is encouraged and supported by the organization.

Qualifications

• Graduate from an accredited School of Nursing.
• Two years of experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing preferred.
• New York State RN licensure preferred.
• Will sit for the CHPN certification within one year of hire.
• Proficient with Word, Excel, and Windows applications.

Responsibilities

• Provide on-call patient visits scheduled or unscheduled for urgent physical and psychosocial issues during after-hours shifts.
• Coordinate with Call Center Triage and On-Call Manager to address urgent patient needs and establish a safe plan of care.
• Educate patients and primary caregivers on changes to plan of care, including medication management and safety.
• Attend patient deaths as directed to provide emotional support and guidance to family.
• Report changes to triage and hospice team and follow up as needed.
• Follow NYS Standards for Professional Registered Nurse.
