Job summary

Concho Hearts Hospice in Pilot Point, TX is seeking an on-call RN to join our team for a 7 days on / 7 days off cycle. The role delivers in-home nursing care across Denton, Pilot Point, Sherman areas, coordinating with the interdisciplinary team and families.

Candidates must have RN licensure, CPR, and experience in home health or hospice; a valid driver's license, reliable transportation, and ability to travel locally; flexibility to work a changing schedule is required.

Qualifications

• Graduate of an accredited nursing program (Diploma/ASN/BSN).
• Current RN license and Driver's license.
• Two years in clinical care; hospice/home health preferred.
• CPR certification; ability to read, write, and understand English.
• Reliable transportation and willingness to travel locally.

Responsibilities

• Administer nursing care for patients in their homes and coordinate with the team.
• Provide end-of-life education and emotional support to patients and families.
• Maintain and update medical charts and health records.
