Job summary

Moments Hospice is seeking an on-call Registered Nurse to organize and direct hospice care in patients' homes. The role emphasizes community health education and family involvement, with nights and weekend coverage as part of a 7 days on / 7 days off schedule.

The position requires RN licensure in the working state, strong nursing assessment skills, and the ability to coordinate with physicians, families, and the interdisciplinary team to ensure safe, compassionate care.

Qualifications

• Must be licensed RN in the state you work in.
• Proficient with Electronic Medical Records (EMR).
• Excellent planning, organizing and communication skills.
• Valid Driver's License and a reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing assessments of patient and family to determine hospice needs.
• Respond to after-hours calls and make visits to patients with changing conditions.
• Provide professional nursing care using the nursing process.
• Write and update the plan of care in collaboration with the physician and family.
