Job summary

Concho Hearts Hospice in Pilot Point, TX is seeking an on-call RN to join our team for a 7 days on / 7 days off cycle. The role delivers in-home nursing care across Denton, Pilot Point, Sherman areas, coordinating with the interdisciplinary team and families.

Candidates must have RN licensure, CPR, and experience in home health or hospice; a valid driver's license, reliable transportation, and ability to travel locally; flexibility to work a changing schedule is required.

Qualifications

• Current RN license and Driver's license.
• Two years in clinical care; hospice/home health preferred.
• CPR certification; ability to read, write, and understand English.
• Reliable transportation and willingness to travel locally.

Job Summary

This is an on-call-only position; it will be a cycle of 7 days on call and 7 days off call. It is a full-time position. Primary functions are to administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient, family, and referring agency; and assume the responsibility for coordination of care.

RN Responsibilities

• Preparing and adjusting patient care plans as needed.
• Providing loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
• Maintaining and updating medical charts, patient files, and health records.

RN Job Qualifications

• Education:
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Licensure:
  • Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), current Driver's License.
• Experience:
  • Two years of experience as a Registered Nurse in a clinical care setting preferred; hospice preferred.
• Skills:
  • Nursing skills as defined by generally accepted standards of practice.
  • Good interpersonal skills.
  • Specialized skills preferred in palliation and end-of-life.
  • Proof of current CPR. Must read, write, and comprehend English.
• Transportation:
  • Reliable transportation.
  • Valid and current auto liability insurance.
• Environmental and Working Conditions:
  • Works in patients' homes in various conditions, with possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
• Physical and Mental Effort:
  • Prolonged standing and walking required, with the ability to lift up to 50 lbs and move patients.
  • Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psycho-social needs.

Transportation

• Reliable transportation.
