Job summary

Qualifications

• Current license to practice as RN in Connecticut.
• Valid driver's license.
• Minimum three years clinical experience in past five years, at least one in palliative/hospice/long-term care.
• Understanding of hospice principles and philosophy.
• Ability to manage terminal care with pain and symptom management.

Responsibilities

• Implements, coordinates and supervises all aspects of delivery of patient care for hospice program.
• Triages incoming calls for referral or care and implements appropriate plan.
• Complies with agency documentation standards.
• Provides consultation and visits for pain management, symptom management, and bereavement care.
• Maintains ongoing communication with IDG staff.
• Calls in report before 8:30 a.m. to all appropriate parties on patient issues/contacts.
• Seeks consultation with other agency staff as indicated.
• Participates in Hospice IDG Team and other meetings.
