Job summary

Waveny Brand is seeking a physician to join our hospice team in Connecticut. The role involves planning and developing patient/family care plans, certifying admissions, and coordinating with the attending physician to manage symptoms and medications.

Responsibilities include acting as a medical resource for the IDG, providing 24-hour physician coverage, and ensuring ongoing quality in hospice services. Strong commitment to hospice principles and team collaboration is essential.

Qualifications

• MD or DO degree and CT license to practice medicine.
• Understanding of hospice principles, psychosocial, spiritual and medical aspects of hospice care.
• At least five years of clinical practice in medicine, family medicine or palliative care (preferred).
• Board certified in internal medicine or family practice.

Responsibilities

• Participates in planning and developing patient/family plan of care with oversight of medications and treatment.
• Certifies and recertifies patient for hospice admission and re-certification.
• Acts as medical resource for the hospice interdisciplinary team, patients, families, and attending physicians.
• Provides 24-hour availability of physician coverage and ensures continuity of hospice medical services.
