Job summary

Choice Hospice, Inc. in Tyler, TX is seeking a Hospice Field RN for evening/night on-call duties.

The role focuses on delivering compassionate, patient-centered care in patients' homes or other settings, coordinating nursing care plans with the IDG, and supporting families through end-of-life care. Responsibilities include assessments, implementing plans of care, educating patients and families, coordinating with Aide/LPN, and maintaining thorough documentation.

Qualifications

• Current RN License
• CPR Certification
• Valid Driver’s License
• Valid Auto Insurance
• At least 2 years of Hospice RN experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on patient’s physical, functional, psychosocial, and environmental needs with documentation and team reports.
• Apply admission and re-certification criteria to establish appropriate levels of care and eligibility.
• Implement and document the plan of care to ensure quality and continuity of care; revise as needed.
• Educate patient and family on disease process, self-care, end-of-life care, nutrition and dietary needs; prioritize needs with IDG; train staff as needed.
• Determine scope and frequency of services based on acuity and caregiver ability; assess needs at admission and throughout care.
• Provide clinical directions to Hospice Aide and LPN/LVN to ensure quality care.
• Prepare progress notes showing progress toward goals and ensure thorough documentation.
• Inform physician and other staff of changes in patient needs and outcomes; monitor response to care.
• Perform on-call responsibilities and provide on-call service as assigned.
