Job summary

Three Oaks Hospice is seeking a dedicated On-Call Registered Nurse (RN) to provide skilled nursing care for patients in their homes. You will coordinate with the interdisciplinary team, patients, families, and referring agencies to ensure seamless care delivery.

Requirements include an accredited Associate or Bachelor's degree in Nursing, an active RN license in the state, CPR certification, and a minimum of two years' clinical RN experience; hospice or home health experience is preferred.

Qualifications

• Active RN license in the state
• Two years RN clinical experience preferred
• CPR certification required
• Strong communication and interpersonal skills
• Proficient computer skills required

Responsibilities

• Provide skilled nursing care to patients in their homes
• Coordinate care with the interdisciplinary team, patients, families and referring agencies
• Administer medications, treatments and assessments as prescribed
• Document care plans and progress in the medical record
