Qualifications

• Active Wisconsin RN license in good standing.
• Hospice, palliative, home health, or strong med-surg experience preferred; training provided.
• Reliable transportation and valid driver''s license.
• Ability to work independently while staying connected to the team.
• Compassion, clinical judgment, and excellent communication skills.

Responsibilities

• Respond to after-hours needs: symptom management, support visits, admissions, and death visits.
• Provide nursing care in patients' homes, assisted living, memory care.
• Educate and support families.
• Collaborate with interdisciplinary team and physicians.
• Help establish strong clinical practices as we grow in Dane County.
