Job summary

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current license as a registered nurse (RN) as per Nurse Licensure Compact
• Two years of experience as a Registered Nurse in a clinical care setting preferred

Responsibilities

• Provide nursing care for patients at their residence.
• Coordinate care with the interdisciplinary group and family.
• Conduct comprehensive assessments of patient needs.
• Implement and revise plans of care as necessary.
• Educate patients and families regarding care and nutritional needs.

Skills

Nursing skills as per accepted standards of practice

Good interpersonal skills

Specialized skills in palliation and end-of-life

Current CPR certification

Fluent in English

Education
