Job summary

MJHS Hospice in Fairview, NJ seeks an experienced RN to provide after-hours on-call visits, coordinate with the triage team, and work with the hospice physician to ensure safe, compassionate care for patients with life-limiting illnesses.

The role requires a graduate from an accredited nursing program, 2+ years in medical-surgical, hospice, or oncology settings, and an active RN license; CHPN within one year is encouraged.

Qualifications

• Graduate from an accredited School of Nursing.
• Two years experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing preferred.
• New York State RN licensure preferred.
• Will sit for the national board certification CHPN within one year of hire.
• Word, Excel, Windows applications.

Responsibilities

• Provide on-call patient visits scheduled or unscheduled after-hours.
• Coordinate with Call Center Triage and On Call Manager.
• Respond via cell phone to triage requests and communicate with the on-call team.
• Educate patient/primary caregiver on changes to plan of care, meds, safety.
• Attend patient deaths to provide emotional support to family.
• Report updates to triage and hospice team and follow up needs.
