Job summary

MJHS Hospice is seeking an experienced registered nurse to provide after-hours on-call visits for urgent physical and psychosocial issues. You will coordinate with the on-call team to address patient needs and establish safe plans of care.

As a graduate of an accredited School of Nursing with NYS RN licensure (NY licensure preferred) and a willingness to pursue CHPN certification within one year, you will support families during life-limiting illness and end-of-life care.

Qualifications

• Graduate from an accredited School of Nursing.
• At least two years of Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing preferred.
• New York State RN licensure and NYS driver's license preferred.
• Willing to sit for CHPN within one year of hire.

Responsibilities

• Provide on-call patient visits during after-hours scheduled or unscheduled visits.
• Coordinate with Call Center Triage and On-Call Manager.
• Educate patient and primary caregiver on changes to plan of care, including medications, safety and symptom management.
• Attend patient deaths to provide emotional support and guidance to family.
• Report changes to triage and Hospice team concerning visits and follow-up needs.
• Follow NYS Standards for Professional Registered Nurse.
