Job summary

MJHS is seeking an experienced hospice nurse to provide on-call visits, after-hours support, and urgent care coordination. You will work closely with the triage team and hospice physician to ensure a safe plan of care and effective symptom management.

Graduate from an accredited nursing school with NY RN licensure; CHPN certification within one year is preferred. Strong communication and teamwork are essential to support patients and families during end-of-life care.

Qualifications

• Graduate from an accredited School of Nursing.
• At least two years experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing preferred.
• New York State RN licensure preferred.

Responsibilities

• Provide on-call patient visits scheduled or unscheduled for urgent physical and psycho-social issues during after-hours shift.
• Coordinate with Call Center Triage and On-Call Manager; respond to triage requests.
• Implement plan of care and educate patient/cayergiver on changes to plan of care, including medications and safety.
