Job summary

Serenity Hospice Pennsylvania is seeking an On-Call Registered Nurse to provide after-hours nursing support for hospice patients in the Wyoming Valley area. The role involves triaging calls, on-call visits, symptom management, and coordinating care with an interdisciplinary team.

The position requires an active Pennsylvania RN license and at least one year of nursing experience, with hospice experience preferred.

Responsibilities

• Respond to after-hours calls from patients, families, and caregivers.
• Conduct on-call visits to address symptom management and urgent patient needs.
• Perform comprehensive nursing assessments and implement appropriate interventions.
• Coordinate patient care with physicians and the hospice team.
• Provide education and support to patients and caregivers regarding hospice services and medications.
• Complete timely electronic clinical documentation.
• Participate in rotating on-call coverage according to schedule.
• Provide patient-centered care reflecting patient and family goals.
