Job summary

Serenity Hospice Pennsylvania is seeking an On-Call Registered Nurse (RN) to support patients and families across Monroe, Northampton, Lehigh, and Berks Counties. The role emphasizes after-hours clinical expertise and compassionate, patient-centered care in the home setting.

You'll work a 7 Days On / 7 Days Off schedule, providing urgent symptom management, nursing assessments, and coordination with the interdisciplinary team. A PA RN license and one year of nursing experience are required.

Qualifications

• Current Pennsylvania RN license.
• Minimum of one year of nursing experience.
• Hospice experience preferred but not required.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and interpersonal skills.
• Compassionate, patient-centered approach to care.
• Valid driver's license and reliable transportation.

Responsibilities

• Responding to after-hours calls from patients, families, and caregivers.
• Conducting on-call visits to address urgent clinical needs and symptom management.
• Performing nursing assessments and implementing appropriate interventions.
• Coordinating care with physicians and the interdisciplinary hospice team.
• Educating patients and caregivers about medications, symptom management, and hospice care.
• Completing accurate and timely electronic clinical documentation.
• Providing compassionate, individualized care that respects each patient's goals and wishes.

WHAT YOU'LL DO

As an On-Call Hospice RN, you'll be a trusted source of clinical expertise, reassurance, and compassionate support for patients and families after hours.
