Job summary

St. Croix Hospice is seeking an experienced Registered Nurse for a 7/7 on-call role. The RN will handle after-hours home hospice visits, admissions, and urgent patient needs according to the plan of care.

Responsibilities include comprehensive patient assessments, updating care plans, administering medications, and supporting families through end-of-life care, while ensuring HIPAA compliance and high-quality compassionate service.

Qualifications

• Must have a current Registered Nurse (RN) license in state(s) of practice.
• Graduate of an accredited school of nursing required.
• Ability to pass background, consumer report and drug test.
• Valid State Driver’s License if driving is required.

Responsibilities

• Respond to after-hours calls and visit patients with changing conditions.
• Complete comprehensive assessment of patient and family to determine hospice needs.
• Develop and revise Plan of Care based on patient status and needs.
• Administer medications and treatments as prescribed in the plan of care.
• Counsel patient and family in meeting nursing needs and care goals.
• Maintain HIPAA compliance and accurate medical records.
