Position Overview:

The Hospice Night On-Call RN provides skilled nursing care, telephone triage, and urgent patient support during designated after-hours shifts throughout Salt Lake County.

This position is focused on responding to the immediate needs of hospice patients and families rather than managing an assigned patient caseload.

This is not a traditional RN Case Manager position.

The Night On-Call RN will respond to after-hours calls, assess patient needs, provide clinical guidance, perform emergency nursing visits when necessary, and may complete emergent hospice admissions.

You’ll work independently in the field while having access to our on‑call nursing team and Administrator On-Call for collaboration, assistance, and support.

The ideal candidate is an experienced, compassionate RN who demonstrates strong clinical judgment, excellent communication skills, and the ability to remain calm and confident during urgent situations.

Key Responsibilities:
After-Hours Triage & Emergency Response:
  • Respond promptly and professionally to after-hours patient and caregiver calls.
  • Assess patient needs over the phone and determine when an in‑person nursing visit is necessary.
  • Provide clinical guidance and education to patients and caregivers regarding symptom management and comfort measures.
  • Perform urgent nursing visits for uncontrolled symptoms, changes in condition, or other immediate patient needs.
  • Assess and manage pain, respiratory distress, anxiety, agitation, and other end‑of‑life symptoms within the RN scope of practice and established orders.
  • Coordinate with hospice physicians and clinical leadership when additional orders or interventions are needed.
  • Provide compassionate support to patients and families during end‑of‑life transitions.
  • Respond to patient deaths and complete appropriate nursing responsibilities, including postmortem care coordination and documentation.
Emergent Hospice Admissions:
  • Complete emergent hospice admissions when needed during on-call hours.
  • Perform comprehensive nursing assessments and initiate appropriate care planning.
  • Coordinate with physicians, families, facilities, and the interdisciplinary team to support timely admissions.
  • Ensure required admission documentation is completed accurately and in accordance with agency procedures.
  • Provide education and support to patients and families during the admission process.
Care Coordination & Team Support:
  • Collaborate with our dedicated team of on‑call nurses to ensure patients receive timely, appropriate care.
  • Communicate with the Administrator On-Call when additional guidance or assistance is needed.
  • Work closely with hospice physicians and the interdisciplinary team to coordinate patient care.
  • Communicate significant changes in patient condition and urgent interventions to appropriate team members.
  • Provide clear handoff communication to daytime clinical staff following overnight events.
  • Coordinate with pharmacies, facilities, physicians, and other healthcare professionals as needed.
  • Maintain professional, compassionate communication with patients, families, and caregivers.
Documentation & Compliance:
  • Complete accurate and timely documentation of telephone triage, nursing visits, admissions, and patient interventions.
  • Follow hospice regulations, agency policies, and professional nursing standards.
  • Maintain patient confidentiality and comply with HIPAA requirements.
  • Ensure all patient care and communication are documented appropriately in the electronic medical record (EMR).
Qualifications:
  • Current, active Registered Nurse (RN) license in Utah.
  • Previous hospice, home health, palliative care, or related nursing experience preferred.
  • Strong clinical assessment and symptom management skills.
  • Experience with end‑of‑life care and comfort‑focused nursing preferred.
  • Ability to work independently and make sound clinical decisions in urgent situations.
  • Excellent communication, critical‑thinking, and problem‑solving skills.
  • Compassionate, dependable, and confident when supporting patients and families during stressful situations.
  • Strong organizational skills and ability to manage multiple priorities.
  • Comfortable providing telephone triage and traveling for urgent patient visits.
  • Experience with electronic medical records and timely clinical documentation.
  • Valid driver’s license, reliable transportation, and ability to travel throughout Salt Lake County.

  • Every other weekend.
  • Responsible for after-hours patient calls and urgent nursing needs during scheduled on-call shifts.
  • Must be available to respond to emergency patient visits and emergent admissions as needed.
  • Travel throughout Salt Lake County is required.
  • Supported by a team of on‑call nurses and an Administrator On-Call.
