Job summary

Ascend Hospice is seeking an RN or LPN On Call Nurse for Lee's Summit, MO. Shift: Fri-Sun, 8 PM to 8:30 AM. Role emphasizes comprehensive hospice nursing, pain and symptom management, and coordination with the care team.

Requirements include a state RN license, hospice/home health experience, strong documentation and communication skills, and ability to travel locally within the territory.

Qualifications

• Must have a State Certified RN license and hospice/home health care experience.
• Excellent documentation skills and ability to work independently.
• Strong communication skills and valid driver's license with reliable transportation.
• Must be available to travel locally within assigned territory.

Responsibilities

• Assumes responsibility for patient visits or on-call schedule, coordinating nursing process phases.
• Provides hospice care focusing on pain control and symptom management.
• Communicates with attending physicians and IDT to coordinate care and resources.
• Maintains up-to-date documentation and charges per agency policy and Medicare rules.
• Administers medications per laws and hospice policy; ensures accurate patient records.
• Assesses home care needs, collaborates with Medical Director on eligibility, and conducts initial assessments.
• Participates in on-call duties and arranges equipment/services as needed.
