Job summary

Ascend Hospice is hiring an RN On Call in Lee's Summit, MO to provide compassionate hospice nursing care. This role involves on-call scheduling, patient assessments, pain and symptom management, and coordination with physicians and the care team.

Must hold a State Certified RN license and have hospice or home health care experience. Strong documentation, communication skills, and the ability to travel locally are required.

Qualifications

• Must have a State Certified RN license.
• Experience in either Hospice or home health care, REQUIRED.
• Excellent documentation skills and the ability to work independently.
• Strong communication skills.
• Valid driver's license & reliable transportation.
• Must be available to travel locally within assigned territory.

Responsibilities

• Assumes responsibility for a patient visit or on call schedule which includes the assessing, planning, implementing and evaluating phases of the nursing process.
• Provides hospice nursing care based on systematic assessment focusing on pain control and symptom management.
• Initiates communication with attending physicians, other hospice staff members and agencies to coordinate care.
• Maintains regular communication with direct supervisor to review assignments.
• Maintains up-to-date documentation at point of service as required by agency policy and Medicare regulations.
• Administers medications in accordance with laws and Hospice policies.
• Maintains current patient records and documents per policies to ensure accurate charges.
• Assesses and collaborates with Medical Director to determine patient eligibility.
• Completes initial assessment of patient and family for home care needs.
• Anticipates end-of-life needs and manages pain/symptoms.
• Re-evaluates nursing needs and uses data to plan care.
• Initiates preventive and palliative nursing procedures.
• Counsels patient and family on end-of-life matters.
• Performs comprehensive death note when appropriate.
• Communicates with physician and IDT about patient needs.
• Participates in on-call duties as defined by policy.
• Ensures arrangements for equipment and services are in place.
