Job summary

Ascend Hospice & Palliative Care MO in Lees Summit, Missouri is seeking an RN On Call (Weeknights) to support patient and family care. Hours are Monday–Thursday, 4:00 PM – 2:30 AM, with duties including symptom management, documentation, and care coordination.

Candidates should hold a current RN license in Missouri, have at least one year of clinical experience, and be capable of working within an interdisciplinary team to ensure high-quality hospice care.

Qualifications

• Hold a Valid and current RN license within the state of operations.
• 1 year of clinical experience.
• Long Term Care/Home Care/Hospice experience preferred.
• Strong verbal and written communication skills.
• Able to work as part of an inter-disciplinary team.

Responsibilities

• Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis.
• Consults with the Hospice Nurse regarding the patient's status and symptom management at appropriate levels.
• Delivers care as prescribed by the attending physician, as supervised by the Hospice Nurse and IDC team.
• Monitors and documents patient/family input and response to care.
• Maintains continuity of patient care through ongoing communication with families and other health care professionals.
• Participates in Interdisciplinary Team Meetings and all other meetings as assigned.
• Maintains sufficient and appropriate home care supplies for patient care management.
• Collection of documentation and paperwork as requested by the Hospice Nurse.
• Assistance with clinical billing as deemed necessary.
• Participates in orientation of new hospice aides as assigned.
