Qualifications

• Must hold a State Certified RN license.
• Experience in Hospice or home health care is REQUIRED.

• Available to travel locally within assigned territory.

Responsibilities

• Performs nursing process including assessment, planning, implementation and evaluation.
• Delivers hospice nursing care focused on pain control and symptom management.
• Initiates communication with attending physicians and IDT to coordinate care.
• Maintains up-to-date documentation at point of service and ensures accurate charges.
• Administers medications per federal/state laws and agency policies.
